Hi, I’m Doctor Jeffrey Thaxton. I frequently get questions about body contouring, abdominoplasty, commonly known as a tummy tuck. So I’ve put together a short video of one actually
happening inmy office. SoI’m going to walk you through it, just kind of describe thesteps, and Ihope you enjoy it.This is afterthe patient had the preoperative markings placed in preoperative holding area and then she was brought to the operating room placed on the operating table and sedated. this is the draping procedureafter she is prepped. This is myassistantgetting surgical instruments in place to start the operation.I am reinforcing the preoperative markings priorto starting making any incisions.This is me adding local anesthesia. As you can see, the patient is already sedated and is not reallyresponding to the needlesticks. I’m now adding tumescent wetting solution, which consistsof acombination of lidocaine and epinephrine to help with pain control and also blood loss. The initial incision ismade with a scalpel.The incisionsare continueddown through the deeperstructureswith an electrosurgicalinstrument,commonly known as a bovee. Thisinstrumentalso controls bleeding as I’m proceeding along with the operation. After making the incision down through the subcutaneous tissue untilthe fascia overlying the deep muscle is encountered,the dissection is thendirected justsuperficial to the muscles uptowards the
ribs in orderto elevate the skin flap offof the underlying muscles. Once the skin flap had been elevated, the redundant skin was excised and discarded.The muscle layer was then sutured vertically inthe midline inorder to flatten the abdomen and also to give a narrowing of the waist to create a more hourglass shape. Once the muscle layer had been tightened up, the skin flap was advanced towards the feet and tacked down tothe underlying muscles with quilting sutures in order to decrease the possibility of getting a fluid collection. Adding the quilting sutures has eliminated the need for drains in order to prevent fluid collections. Once the skin flap had been advanced and tacked down to the level of the umbilicus, the belly button location was then marked. Following the markings for where the belly button was located, an opening in the skin flap was created in order to deliver the umbilicus through the skin flap.The rest of the skin flap was then tacked down to the underlying muscles with more quilting sutures.Next, a layer called the superficial fascia or Scarpa’s fascia was sutured together in order to take tension off of the skin incision.Taking tension off of the
skin incision closure by putting sutures in the deeper structures helps improve the quality of the scar on the skin. The skin around the belly button and along the incision is then closed with sutures.
The skin is closed in two layers and prior to the final layer of skin closure, liposuction is performed on the flanks in order to create the optimal contour. After completing the liposuction and the final layer of suturing, skin glue is then applied to the skin as a dressing. Once the glue has been placed, a compression garment is then applied to the patient and the procedure is complete.
